Stone of San Diego is a non-profit that specializes in the LGBTQ
addiction recovery, mental health and housing programs. Our
Residential Program is an in-patient recovery program with 31 beds,
and a team of counselors, peer support, mental health and
wrap-around services. The Residential Case Manager assists with
completing wrap around services for all program residents and
connecting them to resources, organizations and services they may
need. The Case Manager /SUD Counselor is responsible for assessing
client needs, implementing, monitoring, and evaluating all actions
required to meet the client?s health and human service needs. They
work closely with their fellow resident counselors, LPHA, and the
Program Manager to coordinate high quality care for all residents
entering and transitioning out of the program. We expect this
position to carry a caseload of 15 clients for case management, as
well as, 2-3 clients for weekly SUD counseling. They will also
facilitate a few SUD-focused groups throughout the week. Essential
Functions Develop and manage individualized case management service
plans for 15 residential clients, ensuring clients have the support
and resources they need to succeed. Act as an advocate for clients
across multiple support systems, ensuring timely connection to
appointments and appropriate access to physical and mental health
care, housing, and other essential services. Manage a caseload of
up to 2-3 residents for individual SUD Counseling, meeting with
them a minimum of once a week Document, review, and chart client
progress completing all required paperwork on time Maintain
efficient communication with treatment team through email(s)
Provide screenings, ASAM assessments, and intakes Facilitate
SUD-focused processing group sessions weekly Provide recovery
support through outreach, socialization, and group activities to
support treatment completion. Facilitate transportation for clients
to medical appointments and treatment sessions when necessary.
Attend all internal meetings, and complete all trainings as
required Develop and maintain relationships with community partners
to ensure clients have access to all available resources and
services. Oversee and maintain all confidentiality and privacy
standards for client protected health information and other
information and materials in accordance with agency policies and
procedures, and applicable regulations and laws according to HIPAA.
Maintain accurate records and documentation, adhering to all DMC
standards and agency policies. Other duties as assigned or
necessary to support the program, office and/or the agency Minimum
Qualifications Active registration through CCAPP, CAADE, or CADTP
(Certification is preferred) Ensure timely renewal of registration
Previous Case Management experience Cultural competency and
understanding working in the social determinants of health
impacting LGBTQI communities Current CPR/First Aid Certification
(or within 90 days of hire) ASAM Training (or within 10 days of
hire) Excellent computer skills, including MS Office Suite
Excellent documentation skills Strong verbal communication skills
Ability to work independently and in a group setting Ability to
handle multiple tasks and shifting priorities within a residential
setting Ability to work productively in a fast-paced environment
Ability to meet documentation deadlines according to state, county,
and program needs Ability to work flexible hours - days, nights,
and weekends as needed for a residential setting Valid driver?s
license and reliable transportation Successful completion of a
criminal background check Preferred Qualifications 1 to 2 years
serving clients in a human service setting Bachelor?s degree in
Human Service or related field Previous experience in an AOD
residential setting Case Management experience for a Medi-Cal
funded program Written and spoken bilingual fluency in Spanish and
English Environmental Factors and Conditions/Physical Requirements
While primarily working indoors, counselors also work with clients
outdoors Regularly lift 15 pounds, and occasionally up to 50
