to $40.00 per hour Pay Frequency: 24 Pay Periods Per Year Full Time
Non-Exempt Driving Required Lower end of range will apply to
candidates who meet minimum experience or have limited years of
relevant experience. The higher end of range will apply to seasoned
candidates with considerable years of relevant experience. PATH is
seeking candidates passionate about helping others make a positive
change in their lives to join our Scattered Site Housing Team as
the JMSW Clinical Case Manager. ABOUT PATH Since its foundation in
1984, PATH (People Assisting The Homeless) has pioneered bold and
effective approaches to assist people experiencing homelessness.
PATH operates services throughout California, connecting our
clients to a comprehensive continuum of homelessness prevention,
street outreach, employment preparation and placement assistance,
individualized case management, supportive services, interim
housing, and permanent supportive housing. ABOUT THE JOB Alongside
a team of other case managers, housing locators and community
providers, the Case Manager provides field-based, direct client
outreach, linkage and retention services to people experiencing
homelessness. Case management services aim to promote recovery and
community integration and ultimately help clients break the cycle
of homelessness. Services provided by the Case Manager include, but
are not limited to, individual and group therapy, outreach and
engagement, case assessment and service planning, assistance with
mainstream entitlement, PHA subsidy and other county subsidy
applications, housing location, advocacy and addressing health,
mental health and substance use barriers. Position Responsibilities
include: Perform initial comprehensive biopsychosocial assessment;
develop a plan of intervention that addresses identified needs for
housing, ongoing case management services, substance abuse
treatment, mental health services, and coordination of needed
community services. Collaborate with the client to develop an
individualized treatment plan that addresses short- and long-term
goals. Provide ongoing case management support to assess progress
and ensure treatment plan outcomes are met or adjusted. Provide
field-based therapeutic supportive case management services
addressing mental health, functional, and substance use barriers as
needed to help clients obtain and maintain housing. Consult with
LCSW and supervisor to conduct crisis and suicide risk assessment
and provide appropriate interventions, complete critical incident
reporting as required per contract and PATH policies WHAT YOU BRING

boundaries. PREFERRED QUALIFICATIONS Current and active licensure
or registration with the California Board of Behavioral Sciences
(e.g., ACSW, LCSW) MINIMUM QUALIFICATIONS Master's degree in social
work or equivalent behavioral health degree eligible for licensure
through the CA Board of Behavioral Sciences (e.g., MSW, MFT) One
(1) years of experience in healthcare, case management, homeless
services or working with vulnerable populations MANDATORY
REQUIREMENTS Regardless of qualifications, candidates must: Have

obtain CPR/First Aid training Successfully complete the following
as a condition of hire: Tuberculosis Test Background Screening Drug
Test DRIVING Driving is an essential function of this position. To

of insurance and ownership for personal vehicles used during work
duties The ability to qualify for PATH's insurance coverage
