Under the direction of the Director of Patient Care, the Licensed Vocational Nurse (LVN) provides services to patients in accordance with the State Nurse Practice Act, agency policies, and accepted professional standards of practice utilizing a comprehensive base of nursing theory and nursing process and communicates and documents observations and assessments. What you'll need to accomplish: Ensure quality and safe delivery of home health services to patients/clients and families/caregiversImplement a plan of care initiated by the case managerProvide accurate and timely documentation consistent with the plan of careAssess and provide patient/client and family/caregiver education and information pertinent to diagnosis and plan of careNotify the patient's attending physician, dentist or podiatrist, and other professional persons and responsible staff of significant changes in the patient's conditionPrepare clinical and progress notesAssume responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act, and standards for home health nursingConsistently demonstrate competency with technical nursing skills according to the personal and legal scope of practiceAssessment skills as applied to the patient, family/support system, and environmentTeaching skills according to the patient's needs and within the context of the patient's environment, value system, and physical/emotional statusMedication administrationsInitiate actions to improve the quality of patient careTake appropriate actions independently and responsively in-home care situations.Initiate referrals as needed to other health team members when delegated by the Case ManagerPromote personal safety and a safe environment for patients and coworkersDemonstrate knowledge of safety and infection control practices by compliance with established policies and proceduresDemonstrate safe practice in the use of equipmentAssess the safety of the environment and take initiative to help prevent accidents and promote safetyParticipate in ongoing, professional self-development.Participate in all mandatory education per agency guidelines.Identify needs for personal professional growth.Demonstrates competency in areas of assignment or identifies the need for additional training (pediatrics, infusion, specialized equipment, etc.)Perform related duties as assignedWhat do we require from you?
