Job Opportunity – Case Management RN – Southern California Start Date: ASAP Location: Southern California Compensation: Details provided to qualified applicants Schedule: Day shift; patient ratio 1:25 weekdays, 1:45 weekends; every other weekend required; 40-hour orientation Highlights: Acute care setting with diverse patient population including neonates, adolescents, adults, and geriatrics Caseload of up to 25 patients with interdisciplinary support from physical therapy and social services Epic EMR experience required Business attire or scrubs accepted Up to 7 days of requested time off available No floating requirements between units Requirements: Active California RN license (pending license accepted if obtained by start date) Minimum 2 years acute care case management or discharge planning experience in a hospital setting Previous travel assignment experience required; first-time travelers will not be considered Epic charting system proficiency Strong skills in care coordination, discharge planning, utilization review, and needs assessment Experience arranging DME, SNF placements, and ambulance transport independently Knowledge of HMOs, IPAs, advance directives, POLST, and community resources for vulnerable populations Ability to organize and participate in family meetings and treatment plan development Commitment to work 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) Why It's a Great Opportunity: This position offers an experienced case manager the chance to make a meaningful impact in a well-supported acute care environment.
